Level of Sponsorship
Please select your level of sponsorship for the 2017-18 season:
Gold and Silver Level Sponsorships Only
For Gold and Silver Sponsorships, your logo will be included on a House team jerseys or Rep Division Practice jerseys. If you have a preference on which team you would like to sponsor, please indicate the division and the player/team/coach below. If no team designation or preference is indicated, Spokane Youth Hockey will assign a team sponsorship:
Tournament Sponsorships Only
For Tournament Sponsorships, please indicate the tournament that you would like to sponsor:
You may choose the offline payment option and mail in your sponsorship check. Alternatively, your transaction will be completed through PayPal. A PayPal account is NOT required. You will have the option to choose to pay via credit or debit card or with your PayPal account when you get to the PayPal log-in screen. A successful registration will provide you with a receipt via PayPal. Spokane Youth Hockey will also email a receipt within 24 hours. If you do not receive a PayPal receipt and/or a Spokane Youth Hockey receipt, you will need to contact our office (or re-submit this form). Thank you!
