House Division Registration

Welcome to the registration for the Spokane Youth Hockey House Division. We are excited about the upcoming 2017-18 season and appreciate your interest in our Spokane Youth Hockey House Teams. We encourage you to review our House Division Programming and Evaluation Guide for more information on the upcoming season.

Please note the following USA Hockey Birth Year designations for the 2017-18 season:

Rate Skate Weekend will be September 23-24, 2017 – this is the start of the House Division season where all players will participate in two (2) skates. Coaches will evaluate each player’s skating and hockey abilities over the course of the weekend, and will then draft all players onto our 2017-18 House Division teams. The two (2) Rate Skate times for each division are as follows:

8U Division – Group 1 (Birth Years 2011, 2012 and 2009-10 players who are new to hockey this season)
Saturday, September 23rd from 9:15-10:30 AM on Rink 2 AND
Sunday, September 24th from 9:15-10:30 AM on Rink 2

8U Division – Group 2 (Birth Years 2009 and 2010)
Saturday, September 23rd from 10:45-12:00 Noon on Rink 2 AND
Sunday, September 24th from 10:45-12:00 Noon on Rink 2

10U Division (Birth Years 2007 and 2008)
Saturday, September 23rd from 10:00-11:00 AM on Rink 1 AND
Sunday, September 24th from 10:00-11:00 AM on Rink 1

12U Division (Birth Years 2005 and 2006)
Saturday, September 23rd from 2:30-3:30 PM on Rink 1
Sunday, September 24th from 12:30-1:30 PM on Rink 2

14U Division (Birth Years 2003 and 2004)
Saturday, September 23rd from 2:15-3:15 PM on Rink 2
Sunday, September 24th from 2:00-3:00 PM on Rink 2

18U Division (Birth Years 1999, 2000, 2001 and 2002)
Saturday, September 23rd from 3:45-4:45 PM on Rink 1

Please have your player available during this registration process as there will be sections (below) where your player will need to acknowledge that s/he has read and understands certain procedures and policies. Thank you!

House Division Payment Plan Information

We encourage you to review the House Division Programming Guide for more information on what is included in each Division’s programming for the upcoming season. The season price (below) includes practices (including professional on-ice skating instruction and skills clinics with our Spokane Youth Hockey Director), league and/or division games, your player’s home tournament registration and House Division Division Playoff and Championship games (10U – 18U). Your player’s jersey and socks are also included.

The 2017-18 House Division costs are as follows:

Learn to Play Hockey (Beginner Level Ages 5-9) – $99 (please click the link for this program registration)
8U Division (2009-2012 Birth Years): $450.00
10U Division (2007-2008 Birth Years): $600.00
12U Division (2005-2006 Birth Years): $600.00
14U Division (2003-2004 Birth Years): $600.00
18U Division (1999-2002 Birth Years): $600.00

We offer two payment options:

PAY ONLINE IN FULL: Submit total fees online today upon the conclusion of this registration session via PayPal (credit and debit cards are also accepted). You’re all finished and we’ll see you at Rate Skate!

PAYMENT PLAN: Fees are divided into two (2) installments, the first of which will be collected online today upon the conclusion of this registration session via PayPal (credit and debit cards also accepted). The next payment will be due on or before Rate Skate on September 23, 2017. Your player’s account must be paid in full in order to participate in Rate Skate – no exceptions.

Shortly following your player’s registration today you will be invoiced from Spokane Youth Hockey as paid in full or for the remainder of the payment owed. Please keep that email in order to pay online, or you may make payment by check to our office – no cash payments please.

**Note: Payment plans will only be accepted through September 1, 2017.

Payment Information and Instructions

Your transaction will be completed through PayPal. A PayPal account is NOT required. You will have the option to choose to pay via credit or debit card or with your PayPal account when you get to the PayPal log-in screen. A successful registration will provide you with a receipt via PayPal. Spokane Youth Hockey will also email a receipt within 24 hours. If you do not receive a PayPal receipt and/or a Spokane Youth Hockey receipt, your player has not been successfully registered and you will need to contact our office (or re-submit this form). Thank you!

Please Note: If you are having problems checking out with this form using your PayPal account, please log into PayPal and verify that all payment information is correct for your account. Some users have experienced problems checking out with this form due to PayPal financial information not being updated (i.e. an outdated expiration number).

2017-18 USA HOCKEY NUMBER

In order to continue registering you must have a 2016-17 USA Hockey confirmation number – last season’s numbers are not valid for this tryout. If you do not have a confirmation number, please first register at USAHockey.com then return back to this page to complete your registration with Spokane Youth Hockey.

House Division Jerseys and Socks

Spokane Jr. Chiefs jerseys and socks are provided upon joining a Spokane Youth Hockey House Division team. Please provide your player’s jersey size below


Team Volunteer Responsibilities

Team management is the responsibility of all members of the team. Each family registered with SAYHA will be responsible for volunteering a minimum of three hours during the hockey season. Please indicate below how your family intends to volunteer with the team this season. After team formation, your volunteer interests (below) will be forwarded to the team’s Head Coach for review. Expressing interest in an area does not guarantee that this is where you will serve the team.

New This Season! Spokane Youth Hockey is currently seeking new House and Rep Division sponsors for our teams. If any member secures a new team sponsorship, the Association will credit the member’s family 10% of the sponsorship received! Rep and House Division Team Sponsorship opportunities are available for $500, $750 or $1,000. If you, your workplace or someone you know would like to sponsor a Spokane Youth Hockey team, please click the Team Fundraising/Sponsorship box below and we will be in touch! This is a great way to earn money off of your player’s House Division registration!



USA Hockey Consent to Treat Certification

I certify that on this date, as the parent or guardian of the player that I am registering, that I give my consent to USA Hockey and its medical representative to obtain medical care from any licensed physician, hospital or clinic for the above mentioned participant, for any injury that could arise from participation in USA Hockey sanctioned events.


USA Hockey Player Code of Conduct

1. No swearing or abusive language on the bench, in the rink, or at any team function.
2. No lashing out at any official no matter what the call is. The coaching staff will handle all matters pertaining to officiating.
3. Anyone who receives a penalty will skate directly to the penalty box.
4. Fighting will not be tolerated. Fighting will result in an appearance before a Discipline Committee.
5. There will be no drinking, smoking, chewing of tobacco or use of illegal substance at any team function.
6. I will conduct myself in a befitting manner at all facilities (ice rink, hotel, restaurant, etc) during all team functions.
7. Any player or team official who cannot abide by these rules or violates them will be subject to further disciplinary action.

Waiver of Liability, Financial Acknowledgement and Privacy Policy

Waiver of Liability
By registering with Spokane Americans Youth Hockey Association (SAYHA), the participant and his/her parents or guardians consents to participate at his/her own risk. SAYHA and its owners, employees or agents, shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participant or his/her family in relation to any SAYHA programs. Participants and parents assume full responsibility for all injuries and damages which occur in relation to any programs. He/She does hereby fully and forever release, discharge and hold harmless SAYHA, all associated facilities and its owners, employees, and agents from any and all claims, demands, damages or rights of action, present or future, resulting from any person’s participation in any programs or use of the facility. In addition, he/she agree(s) to follow the rules of conduct and play set by USA Hockey and Spokane Youth Hockey. Failure to do so may result in suspension or expulsion from participation. By registering, you authorize SAYHA and its assigns to utilize any and all photographs, pictures or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in promotional materials or team films. Your registration also authorizes Spokane Youth Hockey to add your email address to our database for email communications (of which you have the ability to opt out or update your preferences at any time).

Financial Obligations
By registering with SAYHA, participant and his/her parents or guardian agree to pay all incurred fees due SAYHA. These fees include payment of all registration fees no later than the dates outlines in the Spokane Youth Hockey House Division Evaluation and Programming Guide and/or the Spokane Youth Hockey Policies and Procedures Handbook. Any returned NSF checks will result in a fee of $25 being added to your account.

Privacy Policy
We take your right to privacy seriously. This Privacy Policy deals with personally identifiable information that may be collected by us on our site. SAYHA collects data provided by you only when the data is voluntarily submitted in forms. If you contact us by email, we may keep a record of that correspondence for future reference. We use the information you provide about yourself and your child when submitting a form only to process the request. We do not share this information with outside parties except to the extent necessary to process the request or for legal process, to establish or exercise our legal rights or defend against legal claims, to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, violations, or as otherwise required by law.
